AFFIDAVIT

Each parent/guardian will be financially responsible for the equipment issued to their child, and will be responsible for the cost of replacing the items lost or damaged due to negligence. Equipment must be ordered early in the year to be available for the next season: therefore, the equipment must be turned in when scheduled. DISCLOSURE

I the undersigned, hereby request permission for my child to participate in activities conducted by the LYFA . I know the risks and dangers involved in tackle football and cheerleading, and that unanticipated and unexpected dangers may arise during such activities. I assume all risks of injury to my child that may be sustained in connection with these activities. I hereby for myself and child release the LYFA and their respective servants, agents, officers, coaches, and all other participants in LYFA activities from all claims, demands, actions and causes of action of any sort, and for injury sustained to my child during my child's participation in the stated activities due to negligence or any other fault other than those claims directed to medical insurance. I have read and understand the foregoing request and release.

INSTRUCTIONS

Print this application fill in the blanks and sign in the appropriate places.

CHEERLEADING

Cheerleading tryouts will be May 4th at the Talley Ward Recreation Center(See Schedule of Events).

Each participant will tryout for the squad in the school jurisdiction they attend or attended if participant has moved on to Middle School. On the day of tryouts each participant must fill out a Participant's Application.  

There are 12 members to each cheerleading squad, and if there are 12 or less participants try out for a squad those 12 or less will automatically be on the squad. Upon making a squad each participant must submit to the LYFA a copy of their state certified birth certificate, this application and the cheerleading fee.

FOOTBALL

Football sign ups will begin May 1st thru July 13th.

Applications submitted prior to May 1st will not be accepted. Each football team allows a maximum of 30 players. There are no football tryouts: therefore, the first 30 players to sign up will be placed on the team.

If you are registering by mail a copy of the above Participant's Application, a copy of their state certified birth certificate and the football fee must be submitted before the applicant will be added to the roster.

Cut-off Dates:  Pee Wee Division-must be six years old on or before August 1st, but cannot be nine years old before October 20th. Junior Division-must be nine years old before October 20th, but cannot be eleven years old before October 20th. Senior Division-must be eleven years before October 20th, but cannot be thirteen years old before October 20th.

FEES

Cheerleading Fee - $40.00    Cheerleading Uniform (Blouse and Skirt) will be provided by the LYFA. Replacement cost if uniform is lost or damaged $120.00

Football Fee- $50.00    Helmet, Shoulder Pads and Game Jersey will be provided by the LYFA. Replacement cost if lost or damaged. Helmet-$70.00.....Shoulder Pads-$40.00 to $130.00 depending upon the size.....Jersey-$60.00

Their will be a $5.00 deduction for each additional participant per family. The fee for coaches children is $15.00 for each child.

*All Christian and Private Schools are free agents and can play for the team of their choice*
