To register, this form must be complete and submitted to the LYFA. Each participant that makes a cheerleading squad after try outs May 4th, or any participant that signs up for a football team beginning May 1st must submit to the LYFA a copy of a state certified birth certificate and the participation fee. The fee for cheerleaders is $40.00 and for football players the fee is $50.00. Cheerleading registration begins April 1st, and Football registration begins May 1st. No registation forms will be accepted before the previous registration dates.
