updated 10/11/11

Registration and Information Night - Thursday November 3rd.

Registration and Information Night will be held the first week of November at the Nissitissit Middle School cafeteria. In addition to registration, the chaperones and I will go over the Program and answer any questions. I recommend all 1st year participants attend the Information Night.

Registration is on a first come, first serve basis so please register early to avoid disappointment. All registration forms must be submitted to the coordinator before the Registration Deadline of November 17th.

If you have any questions about filling out the forms please Contact the Coordinator

Basketball Tryouts:

Please let me know if you are planning to tryout for the High School basketball team but would like to participate in the Ski Program if you don't make the team! It is best to fill out the Ski Program registration forms so I can plan accordingly then, depending on tryouts, you can let me know if you wish to participate.

5th Graders at Nissitissit Middle School:

After discussion with the Pepperell Recreation Board, the decision was made to allow 5 5th grade students for each parent/guardian of a 5th grader who will chaperone, up to a maximum of 15 5th grade students and 3 chaperones.  If your 5th grade child is interested in participating and you wish to be a chaperone please email me for more information.

Drop Off Registration

If you participated in the program last year you do not need to go to the registration night.  Instead you can fill out the forms and drop them off at my house.  I will start to accept forms on October 4.

2) Include cash, check (made out to Wachusett Mountain), or credit card information with the Wachusett Registration form.

3) Mail or drop off the Registration forms in the drop box at my house. (18 Hollis St. Pepperell, corner of Hollis St and Tucker Ave).

There is a Friday Night fee of $10 already included in the Student Badge cost.

If Rentals or Lessons are desired, fill in the appropriate fee.

The 2010-2011 Bus Fee is $90 and is filled in for you (if you are not taking the bus, please talk to the coordinator).

Fill in the Total at the bottom. Payment can be made with cash, check or credit card. Make checks out to "Wachusett Mountain Ski Area" and include the payment with the registration form. Please include 1 check per registration form. If you have 2 students in the program, you need to write a separate check for each.

Medical Information and Treatment Authorization

This form has parent/guardian contact information and medical contact numbers in case your child is hurt at the mountain. All first aid at the mountain is administered by the Ski Patrol and a copy of this information will be provided to them. If your child is hurt, every effort will be made to contact the parent/guardian.

Code of Responsibility

The Program has about one chaperone per fifteen skiers but students are not supervised at all times. Students need to be responsible for themselves, their equipment and be able to ask for help when necessary. This is a list of things the parent/guardian should go over with their child so everyone can have a fun trip. Please return the signature page and keep the Code for your information.

Note the Equipment Discount check box at the bottom of this form. Local ski shops have offered discounts on equipment and services to Ski Program participants. If you request, the student’s name will be included on a list of participants who are eligible for these discounts. The ski shops will have a list of names only and are not allowed to solicit in any way. Each store has slightly different discounts, Contact the Coordinator for the details.
