The required information is your: Name; Street Address; City; State; Zip; Country; and E-mail.  If you do not see all of the boxes to enter this information, please scroll down to view them.  A phone number is not required but is suggested in case we have a problem with your order and need to contact you.  If it is not given, any problems will be handled via e-mail.

If you elect to have your order sent to an address other than the one listed, prior permission may be required by your credit card company.  Typically the credit card companies will not authorize an order if it is to be shipped somewhere other than the actual address given by the credit card holder.

Problems accessing the order processing web page are usually related to security setting issues.  This is especially true if you are using an IE 6 web browser.  Please e-mail us and we can send you a picture of the appropiate settings
