Rock Haven Nursing Home is an important link in Rock County's health and long term care systems.Our goal is to provide services and treatments that maximize an individual's ability to function and attain the highest level of independence. We serve those in need of rehabilitation; the frail elderly; those with behavioral, emotional, and psychiatric needs; and those with developmental disabilities. All units are certified for Medicare and Medical Assistance. Rock County residency is required for admission.

Rock Haven's professional staff is dedicated to providing individualized care to each resident with dignity and respect. Residents are encouraged to maximize their independence by making use of spacious dayrooms and inviting outdoor spaces. At the same time, residents are fully supported in their daily needs including skilled nursing, medical and therapy services, personal care, meals, and activities.

Our Resident Care Units

• Meadow Place: Rehabilitative care, complex medical care, as well as traditional long-term care including intensive and skilled nursing care for frail and medically needy adults.
  
  
• Harbor Way: Structured group activities, psychiatric and behavior interventions, as well as psychiatric and skilled nursing care for persons with gero-psychiatric disorders or severe and persistent mental illness requiring specialized services that many other nursing homes cannot provide.
  
  
• Glen Lane: Intermediate and skilled nursing care for persons with Alzheimer's Disease or other dementias. Activity socialization and life enrichment programs, safety systems, and assistance with all activities of living.
  
  

Each of our units is specalized to privide for the specific needs of our resident population allowing each individual to focus on the things they can do while not having to worry so much about the things they cannot.
