FIRE EXPLOSION- As this gasoline tanker was filling the tanks on the left, its engine began racing due to suction of ground gas fumes into the air filter. This created fire balls out of the stack, igniting an explosion which ultimately engulfed both truck and tanks, causing severe burns to the driver. Faulty vents of an inverted U shape caused vapors to disperse to the ground (shown as a tan colored vapor on the asphalt). Proper 45 degree conical upward vents would aim vapor up into the wind for dispersion as depicted by the concentric circles shown at the top of the image.






RAILROAD ACCIDENT- Regulations require that inoperative signals be bagged and that the rail bed be free of trees obscuring view. The driver of the car approaching this crossing relied on the implicit message of non-bagged, non flashing lights. She could not see the approaching train until the last seconds, and collided with it, causing near death injuries.
